IEPA POLICY

1. MEDICAL AUTHORIZATION
• I HEREBY AUTHORIZE THE BEARER, WHO IS A MEMBER OF THE STAFF OF IEPA, TO SIGN ON MY BEHALF ANY AND ALL FORMS REQUIRED IN ORDER TO OBTAIN EMERGENCY MEDICAL OR HOSPITAL CARE FOR MY CHILD, AND REQUEST THAT NECESSARY EMERGENCY TREATMENT BE PROVIDED BY YOU FOR MY CHILD. I REALIZE THAT I AM RESPONSIBLE FOR PAYMENT OF SUCH EMERGENCY CARE. A PHOTOCOPY OF THIS DOCUMENT SHALL HAVE THE SAME FORCE AND EFFECT AS THE ORIGINAL.

2. AUTHORIZATION AUTO-PAY ENROLLMENT (AUTO-PAYMENT IS REQUIRED)
• I AUTHORIZE IEPA TO CHARGE THE CREDIT CARD OR CHECKING ACCOUNT I HAVE PROVIDED FOR THE STUDENT TUITION.
• I UNDERSTAND THESE CHARGES WILL BE AUTOMATICALLY PROCESSED ON THE 26th OF EVERY MONTH AND THAT IT IS MY RESPONSIBILITY TO UPDATE MY BILLING INFORMATION SHOULD ANY CHANGES ARISE. (PAYMENT FOR MARCH WILL BE FEBRUARY 24TH.)
• I UNDERSTAND THAT IF A MONTHLY PAYMENT CANNOT BE PROCESSED DUE TO OUTDATED BILLING INFORMATION OR OTHER CIRCUMSTANCES AND I FAIL TO UPDATE MY BILLING INFORMATION BY THE END OF THE 1ST WEEK, MANAGEMENT HAS THE RIGHT TO CANCEL THE CLASS OR CLASSES.
• I UNDERSTAND THAT ENROLLMENT RESERVES MY DESIGNATED CLASS INDEFINITELY UNTIL I SUBMIT A REQUEST FOR WITHDRAWAL.
• MANAGEMENT MAINTAINS THE RIGHT FOR CLASS CANCELLATION.
• I UNDERSTAND THAT WITHDRAWAL FROM THE MONTHLY TUITION REQUIRES SUBMISSION OF THE WITHDRAWAL FORM BY THE 15TH OF THE PRECEDING MONTH.
• NO REFUNDS ARE PROVIDED FOR MISSED CLASSES, AND REQUESTS FOR RESCHEDULING MUST BE SUBMITTED AT LEAST 24 HOURS PRIOR TO CLASS TIME ONLINE.

3. IEPA POLICY
• IEPA RESERVES THE RIGHT TO CANCEL PRIVATE TEACHING CLASS IN THE EVENT OF A TEACHER EMERGENCY. WE WILL NOTIFY THE STUDENT OF THE CANCELLATION WITH A FULL REFUND.
• FULL PAYMENT FOR A CLASS IS DUE AT THE TIME OF RESERVATION WITH CREDIT CARD ONLY.
• RESOURCE FEE AND REGISTRATION FEE ARE NON- REFUNDABLE.
• NO REFUNDS OR MAKE UP CLASSES WILL BE OFFERED FOR ABSENCES, LATE ARRIVALS AND/OR WITHDRAWALS AFTER THE START OF THE SESSION.
• CLASS MATERIALS MAY BE PROVIDED BY STUDENT OR PURCHASED THROUGH THE ACADEMY IF NEEDED.
• CLASS TUITION IS BASED ON THE TOTAL NUMBER OF SESSION CLASSES. EACH MONTH MAY HAVE A DIFFERENT NUMBER OF CLASSES.
• CLASS TIME WILL BE STRICTLY MAINTAINED BASED ON RESERVED APPOINTMENT. (CLASS TIME 55 MIN~60MIN)
• STUDENT IS RESPONSIBLE FOR ACADEMY CALENDAR (POSTED ONLINE). HOLIDAYS MAYBE DIFFERENT THAN SCHOOL HOLIDAYS SO PLEASE CHECK THE IEPA CALENDAR (WWW.IEPAONLINE.COM/CALENDAR)
• STUDENTS MAY ONLY BE DROPPED OFF FOR CLASS 10 MINUTES BEFORE THE BEGINNING OF CLASS.
• STUDENTS SHOULD BE PICKED UP ON TIME OR THERE WILL BE A CHARGE.

4. CLASS NOTIFICATION AND CANCELLATION
• [NO. 1] 100% CREDIT AVAILABLE FOR ONLY ONE CANCELLATION FOR THE MONTH. THIS INCLUDES A CANCELLATION REQUESTED BEFORE THE MONTH BEGINS. AFTER 1ST CANCELLATION 50% CREDIT IS PROVIDED FOR FOLLOWING CANCELLATION CLASSES.
• REFUNDS CANNOT BE PROVIDED FOR CREDIT. ONLY TEACHER CANCELLATION CREDIT MY BE REFUNDABLE.
• CANCELLATION REQUEST SHOULD BE A DAY BEFORE THE CLASS BY 5:00 PM. AFTER 5:00 PM CANCELLATION WILL BE COUNTED AS A SAME DAY CANCELLATION.
• CLASS CANCELLATION SHOULD BE VIA TEXT OR EMAIL ONLY. PHONE REQUESTS DO NOT APPLY.
• REQUESTING WITHDRAWAL SHOULD BE BY 25TH BEFORE THE FOLLOWING MONTH. AFTER THE 25TH, CLASS MAY BE CANCELLED BUT REGULAR CANCELLATION POLICY WILL BE APPLIED. (SEE NO. 1 ABOVE)
 REFUDN PROCESSING TIME WILL BE UP TO 30 DAYS FROM THE DAY OF IEPA APPROVAL.
• EXCEPT FOR REFUNDABLE CREDIT, REFUDN PROCESSING FEES (10%, NOT EXCEEDING $20) WILL BE APPLIED.
• IF A STUDENT IS SICK, A DOCTOR’S WRITTEN EXCUSE WILL ALLOW FOR 50% CREDIT FOR SAME DAY CANCELLATION. REQUESTS SHOULD BE SUBMITTED BEFORE 12:00 PM AND DOCUMENTS SHOULD BE SUBMITTED BY 25TH.
• IF A STUDENT IS SICK WITH FEVER, VIRUS OR FLUE ETC, A TEACHER OR MANAGEMENT MAY CANCEL A CLASS.
• IEPA RESERVES THE RIGHT TO CANCEL A CLASS DUE TO CHRONIC NO-SHOW OR LATE CANCELLATION.
• CANCELLATION IS NOT AVAILABLE FOR REGULAR GROUP CLASSES.
• MAKE-UP CLASSES ARE NOT AVAILABLE FOR MISSED CLASSES DUE TO STUDENT ABSENCE. CREDIT IS NOT REFUNDABLE.


5. SUMMER CAMP PROGRAM
•CLASS WILL BE CANCELLED IF THERE IS LESS THAN MINIMUM STUDENT NUMBER. (WILL NOTIFY IN MAY / 100% DISCOUNT)
•CANCELLATION/REFUND IS NOT AVAILABLE AFTER REGISTRATION.
